RPM Care Manager, Healthcare & RPM Programs
Overview
RPM Care Manager owns patient monitoring, engagement, and care coordination for the RPM program at EPIC Health. You’ll monitor vital readings, educate patients, and coordinate timely follow-ups to improve outcomes for chronic conditions. This on-site role works with a multidisciplinary team to ensure HIPAA compliant care and patient satisfaction. You will help patients stay connected to care and participate actively in proactive, value-based care programs.
What You'll Do10
- 1Build daily patient monitoring routines for RPM data such as BP, glucose, weight, and oxygen saturation and drive timely responses to abnormal readings
- 2Design outreach and engagement plans to encourage consistent patient participation in the RPM program
- 3Lead care coordination with physicians, medical assistants, pharmacies, and other team members to escalate high-risk cases
- 4Ship patient enrollment into RPM and educate on device usage, program expectations, and participation requirements
- 5Own documentation of interactions, readings, education, and escalations in the electronic health record
- 6Drive compliance with HIPAA, payer requirements, and organizational policies
- 7Coordinate device inventory management and support RPM device troubleshooting and replacements
- 8Scale patient education and support to improve adherence and engagement metrics
- 9Monitor adherence and participate in team huddles, quality audits, and continuous improvement initiatives
- 10Support clinical escalation pathways and notify licensed personnel when interventions are required
Requirements10
- 1High school diploma or GED required
- 2Experience in healthcare roles such as Medical Assistant, Pharmacy Technician, CNA, Care Manager, Health Navigator, or similar preferred
- 3Experience in primary care, chronic care management, patient outreach, population health, or RPM preferred
- 4Knowledge of chronic conditions such as hypertension, diabetes, COPD, heart failure, obesity, and cardiovascular disease
- 5Strong computer skills and experience with electronic health records
- 6Excellent communication, organizational, and time management skills
- 7Ability to recognize abnormal readings and follow escalation protocols
- 8Current healthcare certification or license preferred
- 9Experience with Athenahealth, ChronicCareIQ, 3CX, or similar platforms preferred
- 10Knowledge of Medicare, Medicaid, value-based care, or RPM programs preferred
