Care Management Enrollment Coordinator, Chronic Care
Overview
Own patient enrollment into our Chronic Care Management program across 30+ locations and 200,000+ patients statewide. Coordinate onboarding with the care management team, driving program growth and patient retention. This role sits at the intersection of patient education and operational execution, offering direct impact on care quality. You will collaborate with providers and care coordinators to streamline transitions, ensuring every eligible patient gets enrolled.
What You'll Do8
- 1Educate patients on Chronic Care Management benefits and secure enrollment during initial outreach.
- 2Collect patient information and documentation, verifying eligibility for CCM services.
- 3Explain program details to patients, addressing questions and easing concerns.
- 4Maintain clean enrollment records in Excel and Outlook, tracking status updates.
- 5Coordinate with care managers to hand off new patients, ensuring a smooth start.
- 6Drive weekly enrollment targets, prioritizing outreach to high-risk patients.
- 7Resolve documentation gaps by following up with patients and providers.
- 8Support the care team with ad hoc administrative tasks during peak seasons.
Requirements7
- 1High school diploma or equivalent.
- 21-2 years in healthcare enrollment, patient intake, or care coordination.
- 3Strong customer service skills, with proven ability to explain complex benefit details.
- 4Proficiency in Excel (pivot tables, VLOOKUP) and Outlook.
- 5Attention to detail, maintaining 99% accuracy in patient records.
- 6Ability to manage 20+ patient calls daily, prioritizing tasks with competing deadlines.
- 7Healthcare experience in chronic care management or Medicare preferred.
