Health Home Enrollment Specialist
Overview
You will own the full enrollment lifecycle for health home services, guiding eligible patients from outreach to activation across New York boroughs. You will work with case managers, providers, and insurance reps to ensure seamless service access. What You'll Do10
- 1Conduct patient outreach and intake for health home programs, ensuring timely and meaningful engagement.
- 2Educate patients and families on health home services, benefits, and eligibility criteria.
- 3Document outreach, eligibility checks, and enrollments in FCM, MAPP, ePACES, PSYCKES, and eCW per BHH, DOH, and OMH standards.
- 4Complete enrollment applications with accurate and compliant documentation.
- 5Verify insurance coverage, Medicaid eligibility, and required authorizations.
- 6Collaborate with case managers and care coordinators to support service activation.
- 7Maintain organized records and update EHR systems.
- 8Submit timely closures for expired or opted-out candidates per BHH policy.
- 9Track progress and report outcomes to the program manager.
- 10Attend community and company events to enroll eligible individuals.
Requirements6
- 1High school diploma or equivalent required; Associate's or Bachelor's degree in health, business, or related field preferred.
- 2Prior experience in health home, healthcare enrollment, intake, or case management support strongly preferred.
- 3Knowledge of Medicaid guidelines, managed care, or community health programs a plus.
- 4Strong communication, interpersonal, and organizational skills.
- 5Proficiency in Microsoft Office and EHR or database systems (ePaces, Health Commerce System (MAPP), eClinical Works (eCW), PSYCKES, Foothold Care Management (FCM)).
- 6Ability to travel throughout the five boroughs for home and community visits via public transportation.
