Overview
An RN Case Manager will own in-home assessments at scale, ensuring accurate OASIS documentation and care plan development for home health clients. What You'll Do10
- 1Build comprehensive in-home assessments that determine care needs and eligibility for home health services
- 2Design and complete OASIS assessments and start-of-care evaluations in line with Medicare guidelines
- 3Lead care planning discussions with physicians, care teams, and families to communicate individualized plans
- 4Docu ment clinical findings accurately and timely in the EHR to support ongoing patient care
- 5Drive regulatory compliance with Medicare, Medicaid, and Joint Commission standards during assessment activities
- 6Communicate patient status changes and clinical concerns to the appropriate care team members
- 7Scale assessment operations across the Arlington, VA service area by coordinating scheduling and travel
- 8Own the assessment process quality, ensuring timely recertifications and re assessments
- 9Monitor and report on assessment outcomes to improve care planning and patient satisfaction
- 10Maintain flexibility to accommodate patient and agency needs in a field-based role
Requirements7
- 1Active and unrestricted RN license in Virginia
- 2Minimum of 1-2 years of home health or clinical nursing experience
- 3Proficiency with OASIS documentation and Medicare home health regulations
- 4Strong clinical assessment and critical thinking skills
- 5Excellent communication, time management, and organizational abilities
- 6Reliable transportation and valid driver''s license for home visit travel
- 7Comfortable working independently in a field-based, patient-facing environment
