Resident Care Manager RN, Senior Nursing Leadership & Care Coordination

What You'll Do9
- 1Build and update care plans based on assessments to achieve person-centered outcomes, coordinating with families and providers.
- 2Design and document treatment plans, orders, and follow-ups using the electronic medical record system.
- 3Lead clinical meetings and SBAR communications to escalate changes in patient condition to physicians.
- 4Collaborate with nursing staff, therapists, and social services to implement plans of care with exemplary service.
- 5Own medical record completeness and accuracy, ensuring current medical condition is reflected.
- 6Coordinate transitions and discharge planning with hospital teams and payors to ensure smooth patient movement.
- 7Educate patients and families about health care plans and care options to support informed decisions.
- 8Ensure punctual attendance and reliable performance for all assigned shifts.
- 9Perform additional duties in accordance with the Code of Conduct and regulatory standards.
Requirements5
- 1RN license current in Oregon
- 2CPR certification current
- 3Degree in Nursing from an accredited school
- 4Six months experience in a long-term care environment preferred
- 5Minimum one year of supervisory experience in a hospital, nursing facility, or related health care setting
