You handle skilled nursing visits for homecare clients across Boston, managing up to 6 visits per day with one-on-one patient focus. What You'll Do8
- 1Assess client health status during home visits, updating care plans in Statewise within 24 hours.
- 2Administer medications and treatments per physician orders, monitoring for adverse reactions.
- 3Educate patients and families on disease management, medication schedules, and safety protocols.
- 4Coordinate with physicians, therapists, and aides to adjust care as client needs change.
- 5Document visit findings in Statewise during each shift, maintaining compliance with state regulations.
- 6Respond to after-hours clinical escalations via phone support, providing guidance to LPNs and aides.
- 7Perform wound care, IV therapy, and ostomy care for clients with complex medical needs.
- 8Participate in weekly care team meetings to review high-risk clients and adjust care strategies.
Requirements8
- 11+ year of clinical experience as an RN in home health, med-surg, or critical care.
- 2Active Massachusetts RN license in good standing.
- 3CPR certification (BLS) required, with ability to renew within 30 days of hire.
- 4Experience with Statewise or similar electronic health record (EHR) systems.
- 5Valid driver's license, reliable car, and car insurance for home visits.
- 6Must pass a background check and drug screening.
- 7Ability to lift up to 50 pounds and assist with patient mobility.
- 8Flexibility to work 1st, 2nd, or 3rd shift, and weekends as needed for coverage.
