Registered Nurse, In-Home Health Assessments
Overview
Conduct in-home health assessments for dually eligible members across Massachusetts, delivering accurate documentation within 48 hours. You own the full assessment cycle from member engagement to clinical handoff, ensuring timely and compliant records. This field-based role operates within a supportive team of nurses and coordinators, backed by Activate Care's Care Link platform. You will specialize in Uniform Core Assessments, Functional Assessments, and Comprehensive Assessments, reporting directly to the health plan's care management team. The role offers flexible scheduling and a Sign-on Bonus for full-time and part-time hires, with a focus on patient-centered care.
What You'll Do10
- 1Engage with members in homes and community settings, tailoring communication to cultural and linguistic needs.
- 2Perform Uniform Core Assessments, Functional Assessments, and Comprehensive Assessments for each assigned member.
- 3Document assessments in the GuidingCare platform following provided workflows and resource guides.
- 4Send a SOAP note to the assigned health plan Care Manager after each assessment.
- 5Submit complete documentation within 48 hours, meeting NCQA and regulatory standards.
- 6Complete required training and maintain system access for ongoing assessments.
- 7Travel routinely to member homes and community settings, following field-safety practices.
- 8Report abuse, neglect, or exploitation as a mandated reporter per state law.
- 9Adhere to HIPAA and privacy regulations, protecting member information.
- 10Maintain active RN licensure and submit to driver's license and insurance checks.
Requirements10
- 1Active Registered Nurse license in Massachusetts with good standing.
- 2Associate of Science in nursing from an accredited program; BSN preferred.
- 31+ year of clinical experience in a home health setting.
- 41+ year of experience completing health assessments.
- 5Prior experience with Medicaid or Medicare managed care preferred.
- 6Valid driver's license, personal vehicle, and verifiable insurance.
- 7Comfortable working independently and managing a daily visit schedule.
- 8Proficiency with secure virtual desktop and electronic documentation platforms.
- 9Basic computer skills with standard office software.
- 10Willingness to complete influenza vaccination during flu season (Oct-Mar).
