Overview
We seek a Registered Nurse to deliver skilled nursing care at patient residences under clinical supervision. The ideal candidate will own patient care delivery and coordinate with multidisciplinary teams. This role offers competitive compensation and benefits.
What You'll Do11
- 1Administer skilled nursing care per physician orders and plan of care
- 2Conduct initial evaluations and ongoing nursing assessments
- 3Initiate and modify care plans based on patient needs
- 4Provide specialized nursing interventions and preventive care
- 5Document clinical notes and care conference summaries promptly
- 6Coordinate services with therapists and social workers
- 7Participate in agency training and quality improvement initiatives
- 8Maintain patient confidentiality and adhere to HIPAA regulations
- 9Travel to patient homes as scheduled
- 10Supervise and mentor junior nursing staff
- 11Process medical documentation and certify patient status changes
Requirements11
- 1Graduate degree in nursing from accredited program and current RN license
- 2Minimum 5+ years nursing experience with home health settings
- 3Valid driver’s license and ability to lift 25 pounds
- 4Proficiency in CPR and First Aid certification
- 5Strong communication skills and ability to work independently
- 6Bilingual English proficiency preferred
- 7Excellent organizational skills and attention to detail
- 8Ability to pass CORI background check
- 9Physical stamina for standing and moving throughout the day
- 10Experience with ICD-10 coding systems
- 11Commitment to patient-centered care and ethical practice
