What You'll Do10
- 1Provide broad range of infusion therapies in patients homes and Ambulatory Infusion Suite
- 2Initiate develop and implement nursing plan of treatments
- 3Assess patient needs and physical status at each visit using health data systems
- 4Coordinate discharge planning and prepare discharge summaries with patient instructions
- 5Communicate with clinical team medical providers patients and families to facilitate continuity of care
- 6Prepare clinical documentation in real time during visits and submit to nursing department
- 7Observe HIPAA guidelines and safeguard confidential information
- 8Participate in Performance Improvement Program and assist with chart audits
- 9Maximize work efficiency using computers and automation technologies to validate orders and visit records
- 10Travel within West Valley Phoenix metro area and surrounding areas with paid travel time and mileage
Requirements5
- 1Registered Nurse with current unrestricted license in Arizona
- 2Minimum two‑year nursing experience IV related experience a plus
- 3Current CPR certification
- 4Valid driver's license and auto insurance
- 5Experience with Microsoft Office Excel Outlook and Word to track patient care activity
