Infusion Nurse, Home Infusion Therapy
Overview
You will own patient care for complex infusion therapies in the home setting, managing a caseload across the Grayson, GA area. You will work with a multidisciplinary team including pharmacists, prescribers, and dietitians to deliver care. This role requires a current RN license and at least 3 years of nursing experience. You will participate in an on-call rotation and drive up to an hour to patient homes. This position offers mileage reimbursement and paid drive time, plus a supportive company culture focused on simplifying complex care.
What You'll Do9
- 1Provide direct care for assigned patients, initiating and maintaining communications with pharmacists, prescribers, dietitians, and care partners.
- 2Evaluate and educate patients and care partners on safe administration of home infusion therapy, providing additional instruction as needed.
- 3Assess the home environment for safety and identify interventions to render it appropriate for prescribed care.
- 4Develop the plan of care with team members and evaluate patient response to therapy at each encounter.
- 5Manage your schedule via the company-approved platform, communicating changes promptly.
- 6Coordinate patient care through the approved platform and participate in the nurse on-call rotation.
- 7Document all patient encounters, communications, and care activities at time of service per policy.
- 8Maintain strong knowledge of Soleo therapies, protocols, and clinical standards, ensuring timely submission of plans of treatment.
- 9Work with departments to complete required patient documents like consents and POTs within acceptable timeframes.
Requirements9
- 1Current RN license by the applicable state’s Board of Nursing, in good standing.
- 2RN degree; BSN from an accredited institution preferred.
- 3Participation in on-call rotation, including nights and weekends.
- 4Provide administrative oversight, including monitoring workflows and ensuring compliance.
- 5Available to respond to emergencies promptly, including non-standard hours.
- 63+ years nursing experience, with infusion therapy background preferred.
- 7Experience in ICU or Cardiac Care preferred.
- 8Proficient with Microsoft Outlook and Microsoft Office Suite.
- 9Valid state issued driver’s license with no significant restrictions.
