Home Infusion Nurse, RN - Patient Care in Home Infusion Therapy
Overview
Own patient-centered care delivering home infusion therapy with independence and clinical judgment at scale. Collaborate with families and healthcare teams to coordinate infusion care across home, hospital, and clinic settings. Maintain rigorous documentation, HIPAA compliance, and exceptional service in a dynamic territory-based role. What You'll Do10
- 1Build and coordinate all aspects of infusion therapy including preparation, administration, and monitoring to ensure safe, effective treatment
- 2Deliver patient-centered care in collaboration with patients, families, and the healthcare team to achieve high-quality outcomes
- 3Provide comprehensive patient and family education on infusion therapy, side effects, and self-care, and document teaching
- 4Complete all required documentation including assessments, visit notes, and encounter records within established time frames
- 5Participate in interdisciplinary collaboration by communicating urgent clinical findings and changes in patient status to the team
- 6Maintain clinical competencies and stay current with best practices in infusion therapy and home care
- 7Protect privacy and confidentiality of PHI and ePHI in accordance with HIPAA and laws
- 8Operate safely in patients' homes and various clinical settings while managing equipment and supplies
- 9Travel within the assigned territory to perform home visits and infusion therapy as needed
- 10Assist with on-call responsibilities to respond to urgent patient needs outside regular hours
Requirements10
- 1Current active RN license in the state of employment
- 2Experience administering infusion therapy in a home, hospital, or clinical setting
- 3Strong IV skills with peripheral and/or central line management
- 4Current CPR certification
- 53–5 years of experience in homecare preferred
- 6Valid driver’s license and ability to travel within the territory
- 7Ability to lift up to 10 pounds and operate a vehicle in adverse conditions
- 8Proficiency in clinical judgment and critical-thinking for patient assessment and care decisions
- 9Strong documentation skills to complete assessments and notes on time
- 10Commitment to patient confidentiality and HIPAA compliance
