What You'll Do6
- 1Build and follow a designated care plan for patients in home settings to ensure needs are met
- 2Perform home visits across geographic territories, administer medications, wound care, and other procedures
- 3Monitor patient conditions and report changes to Clinical or Client Services Manager
- 4Follow up on doctors orders, document actions accurately, and coordinate care activities
- 5Drive case management and care coordination, including documentation of observations and interventions
- 6Document observations, interventions, and evaluations in EMR or touch pad tablet for compliant patient records
Requirements7
- 1Hold a current VA Registered Nurse license
- 2Minimum of 1+ year of recent clinical (medical/surgical) experience
- 3Prior home care experience preferred but not required
- 4Graduate from an accredited nursing program with transcript or diploma
- 5Ability to work independently and manage time effectively
- 6Strong interpersonal skills and solid computer skills with EMR experience preferred
- 7Willingness to travel to cases as assigned
