Overview
21HHS seeks a skilled Field Registered Nurse to deliver patient care across multiple counties while supporting operational efficiency. This entry-level RN role focuses on developing fundamental nursing skills within a home health setting. The position reports to a Case Manager Registered Nurse and contributes to a growing network of healthcare professionals.
What You'll Do11
- 1Provide hands-on patient care including PICC lines wound vacuums kangaroo feeds NG G J tubes staple suture removal drains JP biliary nephrostomy Foley suprapubic catheters trach care blood draws ostomy management SQ and IM injections IV antibiotics TPN compression wraps
- 2Implement a plan of care initiated by the Case Management Registered Nurse
- 3Document patient information accurately within 24-48 hours
- 4Educate patients and caregivers about diagnoses and treatment plans
- 5Assist in skilled procedures and prepare treatment materials
- 6Coordinate home health services with other disciplines such as HHA OT PT MSW ST
- 7Utilize equipment efficiently and participate in professional development
- 8Attend weekly case conferences and provide timely reports
- 9Retrieve patient communications and establish care priorities
- 10Maintain confidentiality and adhere to professional standards
- 11Complete start of care assessments and participate in on-call duties
Requirements8
- 1One year of recent nursing experience with preference for one year in home health care
- 2Experience with high acuity patients and interdisciplinary teamwork
- 3Graduate of an accredited nursing school Bachelor's degree
- 4Current CA Registered Nurse License and BLS certification
- 5Valid CA drivers license with acceptable driving record
- 6Strong communication critical thinking and organizational skills
- 7Computer literacy and ability to perform basic math
- 8Commitment to annual compliance training and certifications
