Registered Dietitian Heart Transplant Phoenix
Overview
You will own comprehensive nutrition care for patients with advanced heart failure, pre/post heart transplantation, and mechanical circulatory support devices like LVAD and TAH at Banner University Medical Center Phoenix. You will join the multidisciplinary transplant team, collaborating with medical providers, nursing, pharmacists, and culinary staff. Your work directly optimizes patient clinical status and supports recovery. This role offers a Monday through Friday schedule with no weekends or holidays.
What You'll Do9
- 1Conduct nutritional assessments and nutrition-focused physical exams for a complex caseload of heart failure and transplant patients, using evidence-based standards.
- 2Develop and manage individualized nutrition care plans, including enteral and parenteral nutrition protocols, and write orders per state regulations.
- 3Perform frailty screenings and contribute to optimization of patient clinical status before and after surgery.
- 4Educate patients and families on nutrition for heart failure, transplant, and mechanical support, tailoring materials to individual needs.
- 5Collaborate with the interdisciplinary team to evaluate care plan effectiveness and adjust based on patient outcomes.
- 6Develop nutrition standards of practice and educational materials in collaboration with peers.
- 7Participate in quality improvement activities and outcome studies to enhance nutrition care delivery.
- 8Serve as a mentor to new staff and interns, and provide in-service education to the broader healthcare team.
- 9Monitor and improve patient care procedures as a liaison with nursing areas and process improvement teams.
Requirements9
- 1Bachelor's degree in Nutrition, Dietetics, or equivalent coursework from an accredited program.
- 2Registered Dietitian or RD Eligible with credentials from the Commission on Dietetic Registration within 6 months of hire.
- 3Current registration with the Commission on Dietetic Registration and ongoing annual verification.
- 4Licensure as required by state law.
- 5Cardiac experience with heart failure and/or transplant experience highly preferred.
- 6Strong patient education and assessment skills, with excellent communication and collaboration abilities.
- 7Ability to work independently and as a team player while serving as a mentor and resource.
- 8Effective prioritization of assigned patient caseload.
- 9For behavioral health settings, Arizona Fingerprint Clearance Card required at hire.
