Overview
Lead patient care and nutrition management for bariatric surgery patients in a fast-paced environment. Provide comprehensive nutritional assessments and individualized care plans focused on long-term success. Manage the care continuum through counseling and ongoing monitoring. Lead patient education initiatives and collaborate with multidisciplinary teams to ensure accurate documentation and compliance.
What You'll Do5
- 1Conduct comprehensive pre- and post-operative nutritional assessments and manage individualized care plans for long-term bariatric success
- 2Provide nutritional counseling focusing on diet progression supplementation and long-term habit formation
- 3Facilitate bariatric education classes host support groups and teach mindful eating and sustainable behavior modification
- 4Partner with surgeons and multidisciplinary teams while ensuring accurate compliant medical record documentation
- 5Maintain professional licensure and meet organizational standards throughout employment
Requirements5
- 1Registered Dietitian/Nutritionist credential from Commission on Dietetic Registration prior to hire date
- 2High school diploma equivalency with two years cumulative experience or associate degree technical degree or four years relevant experience
- 3Outpatient experience with cardiac patients
- 4Experience working within Ascension facility
- 5Commitment to collaborative care model emphasizing service growth and connection
