The PT Direct Care Professional will own resident care and rehabilitation outcomes at our Philadelphia facility. This full-time position requires a compassionate caregiver dedicated to supporting children with intellectual and physical disabilities through personalized daily living assistance and therapeutic engagement. What You'll Do8
- 1Assist residents with ADLs and meal preparation
- 2Transport patients to medical appointments and community events using company vehicles
- 3Implement therapeutic activities including physical exercises and recreational games
- 4Maintain accurate daily documentation of resident progress and needs
- 5Provide companionship and maintain respectful communication with all stakeholders
- 6Report health status changes promptly to supervisory staff
- 7Support residents during emergencies and assist with hobby-based activities
- 8Collaborate with multidisciplinary teams to implement individualized care plans
Requirements11
- 1High School Diploma or GED required
- 2Valid driver's license with 5 years driving experience
- 31-3 months relevant experience or equivalent education/training
- 4Ability to lift and move up to 100 pounds with assistance
- 5Mental fortitude to manage emotionally challenging situations
- 6Certification in CPR and First Aid preferred
- 7Background check clearance
- 8Previous home health aide or nursing assistant experience valued
- 9Strong written communication skills
- 10Ability to work flexible hours including weekends
- 11Commitment to inclusive practices and trauma-informed care
