Overview

Public Health Solutions (PHS) seeks a Project Manager, Health Systems to lead health systems engagement strategies across Bronx Manhattan and Staten Island. This grant-funded role ends April 30 2031. What You'll Do20

  • 1Understand and implement the NYS Department of Health Tobacco Control Program’s strategic plan while supporting PHS’ tobacco control strategic plan
  • 2Partner with CHCs FQHCs LGBTQIA+ Health & Human Services Network and systems serving people with serious mental illness and substance use disorders
  • 3Collaborate with NYC Treat to Quit staff to achieve region specific work plan deliverables
  • 4Work with clinical leadership to implement tobacco cessation policies and protocols including the Ask-Advise-Refer model
  • 5Develop or optimize EHR prompts clinical decision support tools and referral pathways
  • 6Train and coach staff on tobacco screening brief intervention and cessation counseling
  • 7Collaborate with hospitals FQHCs behavioral health systems and local coalitions to support tobacco-free policies
  • 8Promote long term integration of tobacco dependence treatment practices within clinical workflows
  • 9Lead outreach training and education activities with healthcare systems clinical leadership and stakeholders
  • 10Coordinate health system activities including provider trainings and quality improvement sessions
  • 11Assist clinical leadership in monitoring program implementation and identifying quality improvement strategies
  • 12Utilize communication channels to engage leadership administrators and policymakers through educational materials and newsletters
  • 13Ensure outcome measures are achieved and report accurately on activities and outcomes
  • 14Meet with elected officials and policy stakeholders to support program visibility and tobacco control priorities
  • 15Maintain documentation of program activities and outcomes in required reporting formats
  • 16Collect and synthesize program data related to outreach training and cessation outcomes
  • 17Attend mandatory local regional and state meetings and participate in state program workgroups
  • 18Develop robust health systems engagement strategy aligned with organizational goals
  • 19Participate in yearly data collection projects assigned by the state program
  • 20Stay current on tobacco control issues and all assigned duties

Requirements13

  • 1Bachelor’s degree or higher in a related field
  • 2At least 3 years of experience integrating public health into healthcare and behavioral health systems
  • 3Proven experience with policy and community health interventions partnership building
  • 4Knowledge of tobacco cessation best practices and emerging control issues
  • 5Excellent oral and written communication skills
  • 6Strong computer skills and ability to coordinate multiple tasks with competing priorities
  • 7Ability to engage with healthcare partners during non traditional work times
  • 8Demonstrated skills in adult learning principles and structured training facilitation
  • 9Flexibility to travel within NYC boroughs across NYS and nationally for meetings
  • 10Residence in the tri state area by confirmed start date
  • 11Independent work style and teamwork capability
  • 12Familiarity with NYC communities and neighborhoods
  • 13Willingness to travel across the nation for trainings and conferences when eligible
