Overview

PrEP retention coordinator will own patient education, engagement, and retention efforts across multiple Philadelphia FIGHT clinics. They will support PrEP care within the Broad Street Love clinic and coordinate resources at JBHC and Y-HEP, expanding outreach to at-risk populations. Youll partner with clinical teams to develop education, navigation, and adherence strategies and participate in community outreach events that reduce HIV health disparities. This role offers cross-site collaboration and hands-on program growth within a mission-driven health network.

What You'll Do10

  • 1Build and maintain PrEP patient engagement workflows that reduce gaps in care
  • 2Lead education on HIV prevention strategies and adherence, with outcomes showing improved PrEP initiation and continuation within 90 days
  • 3Design intake and risk assessment protocols to identify individuals at increased vulnerability to HIV and connect them to PrEP
  • 4Ship outreach activities and reminders to patients via calls, texts, and visits, ensuring timely dosing and lab appointments
  • 5Own documentation in the EMR for all patient encounters within 48 hours and coordinate with pharmacy for PrEP distribution
  • 6Promote linkage-to-care by coordinating referrals and ensuring timely appointment scheduling across BSL JBHC and YHEP
  • 7Drive data tracking for active PrEP patients, overdue labs, and lost-to-care cases to re-engage individuals
  • 8Collaborate with program managers to cultivate community partnerships and participate in health fairs and outreach events
  • 9Support rapid HIV testing and STI screening as part of PrEP care and prevention services
  • 10Coordinate TelePrEP scheduling and at-home lab logistics to streamline care

Requirements10

  • 1High School Diploma required
  • 2Bachelor's Degree preferred
  • 31-2 years conducting non-medical assessments and client centered education preferred
  • 4PA HIV Testing Certification preferred
  • 5Strong computer literacy with Microsoft Office and EMR experience preferred
  • 6Knowledge of PrEP and HIV prevention and treatment
  • 7Proven organizational skills with attention to detail and responsiveness
  • 8Ability to work across teams with culturally sensitive communication
  • 9Sensitivity to cultural, economic, gender, and sexual orientation differences
  • 10Demonstrated commitment to reducing health disparities and serving vulnerable populations
