Own comprehensive case management for clients living with HIV/AIDS at Bebashi - Transition to Hope, a Black-founded nonprofit serving Philadelphia since 1985. What You'll Do9
- 1Complete a face-to-face comprehensive needs assessment for each new client within the required timeframe.
- 2Develop and maintain individualized service coordination plans with client-defined goals targeting ongoing medical needs.
- 3Enroll eligible clients in the 340B program and provide adherence counseling to ensure medication consistency.
- 4Coordinate referrals for transportation, emergency resources, housing, medical care, support groups, counseling, and nutritional assistance.
- 5Monitor service effectiveness and reassess client needs on a continuous basis.
- 6Facilitate individual counseling sessions and support groups as needed.
- 7Document all client interactions and maintain records in compliance with HIPAA and agency policies.
- 8Submit monthly statistical and time management reports to the Director of Clinical Services.
- 9Participate in ongoing training, conferences, and agency events to stay current with professional standards.
Requirements8
- 1Master's degree in social work or related human service field, or bachelor's degree with 2+ years of case management experience.
- 2Experience working with individuals living with HIV/AIDS strongly preferred.
- 3Demonstrated ability to communicate sensitively and clearly both orally and in writing.
- 4Ability to work effectively as part of a multi-disciplinary team.
- 5Capacity to work autonomously while responding to supervision.
- 6Willingness to travel throughout the city, including economically depressed areas.
- 7Proficiency in using standard office equipment and Microsoft Office for documentation and reporting.
- 8Must be able to lift and carry up to 20 pounds occasionally and navigate varied environments.
