What You'll Do9

- 2Create individualized plans addressing physical social spiritual cultural educational and age specific needs
- 3Implement skilled treatments and modify plans based on progress assessments
- 4Document evaluations using professional practice guidelines within required timeframes
- 5Communicate patient progress to physicians nurses families and interdisciplinary teams
- 6Supervise PTA ATC therapy technicians and students ensuring competency before delegation
- 7Participate in process improvement initiatives and attend required department meetings
- 8Manage daily operations including scheduling and adherence to productivity standards
- 9Collaborate with hospital networks and physicians for integrated care solutions
Requirements5
- 1Graduated from APTA accredited physical therapy program or equivalent
- 2Current physical therapy license in Arizona with fingerprint clearance
- 3BLS certification and proficiency with patient care software and equipment
- 4Minimum 5+ years experience with home health clinical environments
- 5Strong communication skills for patient family interactions and interdisciplinary coordination
