Overview

The Physical Therapist will own patient assessment and treatment planning at scale across clinics in the Phoenix area. You will build personalized rehabilitation plans, supervise interventions, and monitor outcomes while collaborating with a multidisciplinary team. This role focuses on delivering compassionate, evidence‑based therapy and adapting to schedule changes in a patient‑centered environment.

What You'll Do10

  • 1Build comprehensive patient assessments and treatment plans to achieve functional goals
  • 2Deliver manual therapy and therapeutic exercises to improve mobility and strength
  • 3Monitor progress, adjust plans to optimize outcomes, and document progress notes
  • 4Educate patients and families on home exercise programs and injury prevention
  • 5Collaborate with physicians, nurses, and other clinicians to ensure coordinated care
  • 6Delegate tasks to PTA when applicable and support nonclinical team members
  • 7Maintain timely documentation of evaluations, progress notes, and discharge summaries
  • 8Stay current on PT techniques and technologies to enhance practice
  • 9Adapt to changing patient schedules and service needs
  • 10Support a collaborative, mission‑driven culture across the clinics

Requirements6

  • 1Valid state licensure as a Physical Therapist
  • 2Doctorate or Master’s degree in Physical Therapy from an accredited program
  • 3CPR certification and state‑required certifications
  • 4Strong clinical skills in orthopedic, neurological, or geriatric PT (preferred)
  • 5Excellent communication and teamwork skills
  • 6Proficient computer skills for documentation and record keeping
