Physical Therapist PRN, Skilled Nursing & In-House Rehab

You will own physical therapy delivery for short-term and long-term residents at Park Manor of Cyfair, a skilled nursing facility in Houston, TX. You will join an in-house rehab team of PTs, OTs, and SLPs under HealthMark Rehabilitation, treating patients with orthopedic, neurologic, and geriatric conditions. This PRN role offers weekday and weekend shifts in a facility with state-of-the-art equipment and a large interdisciplinary team.

We seek a PRN Physical Therapist to provide skilled therapy services at our Green Oaks facility in Arlington Texas. This role supports skilled nursing facility patients with diverse needs. You own the full therapy caseload as case manager, restoring function and keeping patients safe at home across the Houston area. You work with a team of RNs, MDs, and PTAs to drive real outcomes for high-acuity patients. You will own the full rehabilitation process for complex medical patients at Houston Medical Center – AMG Specialty Hospital, a Long-Term Acute Care facility. You will evaluate each patient's potential, set realistic goals, and direct a treatment plan that respects their functional limits. Working within a multi-disciplinary team, you will coordinate with nurses, physicians, and therapists to push recovery beyond standard hospital care. This PRN role offers flexibility in scheduling while delivering high-acuity care that traditional settings cannot match.
