Physical Therapist PRN, Skilled Nursing & In-House Rehab
Overview
Own patient mobility and independence as a PRN Physical Therapist in our in-house rehab team at a skilled nursing facility in Houston TX. You will evaluate residents, design personalized treatment plans, and deliver interventions to improve mobility, strength, balance, and functional independence. Collaborate with an interdisciplinary team to drive resident outcomes in a supportive, onsite setting.
What You'll Do9
- 1Build and implement individualized treatment plans using evidence based practices to improve mobility and reduce pain
- 2Assess residents and monitor progress to adjust therapy plans within 90 days to achieve measurable gains
- 3Educate residents families and caregivers on exercises mobility strategies and safety
- 4Document accurately and timely per facility and regulatory standards
- 5Collaborate with the interdisciplinary team to ensure optimal resident outcomes
- 6Deliver consistent therapy services in an in house program with clinical standards
- 7Maintain compliance with safety and regulatory requirements
- 8Provide therapy services onsite in a skilled nursing facility setting
- 9Contribute to continuous improvement of therapy processes and documentation
Requirements5
- 1Active Texas Physical Therapist PT license in good standing
- 2SNF or long term care experience preferred
- 3Familiarity with Net Health documentation system a plus
- 4Strong communication and clinical skills
- 5Commitment to weekend availability and PRN schedule
