Physical Therapist, Home Health in Houston & Intermittent Travel

What You'll Do10
- 1Build comprehensive treatment plans with physicians and the care team to meet patient goals
- 2Assess patient function and progress through regular evaluations and documentation
- 3Collaborate with family and caregivers to support discharge planning
- 4Provide interventions per the plan of care and professional standards of practice
- 5Communicate with the interdisciplinary team and physicians to coordinate care
- 6Document treatment, patient response, and progress in a timely and accurate manner
- 7Participate in QAPI programs and ongoing in-services
- 8Supervise PT staff and assistants as assigned
- 9Engage in emergency preparedness and office training as requested
- 10Maintain CPR certification and clinical competencies
Requirements10
- 1Current licensure as a physical therapist in the applicable state
- 2Two years of physical therapist experience in the last five years preferred
- 3Proof of CPR certification
- 4Basic computer and software proficiency
- 5Ability to communicate effectively in English with all levels of staff and caregivers
- 6Ability to read and interpret technical instructions for patient care and equipment
- 7Valid driver’s license and auto liability insurance if driving to patient locations
- 8Ability to travel within the service area and work flexible hours
- 9Strong clinical judgment and ability to manage multiple tasks
- 10Demonstrated ability to work under stress and adapt to changing patient needs
