Physical Therapist PRN - Houston LTAC Hospital
Overview
You will own the full rehabilitation process for complex medical patients at Houston Medical Center – AMG Specialty Hospital, a Long-Term Acute Care facility. You will evaluate each patient's potential, set realistic goals, and direct a treatment plan that respects their functional limits. Working within a multi-disciplinary team, you will coordinate with nurses, physicians, and therapists to push recovery beyond standard hospital care. This PRN role offers flexibility in scheduling while delivering high-acuity care that traditional settings cannot match.
What You'll Do8
- 1Assess new patients within 24 hours of admission to determine rehabilitation potential and baseline functional status.
- 2Design individualized treatment plans that target mobility, strength, and balance using evidence-based modalities.
- 3Collaborate with the care team to adjust goals weekly based on patient progress and tolerance.
- 4Document every treatment session in the EMR within the same shift to ensure accurate care continuity.
- 5Educate patients and families on home exercise programs and safety precautions for a smooth transition.
- 6Supervise PTAs and support staff during dual-treatment sessions to maintain quality standards.
- 7Participate in discharge planning meetings to recommend durable medical equipment or follow-up therapy.
- 8Handle a caseload of 6-8 patients daily in an LTAC setting with a 1:5 staff-to-patient ratio.
Requirements6
- 1Current Physical Therapy License in Texas.
- 2Current BLS certification.
- 32+ years of clinical experience, preferably in acute care or LTAC.
- 4Fluency in English; bilingual skills a plus.
- 5Basic computer literacy for EMR documentation.
- 6Ability to perform physical assessments and interventions for complex, multi-morbid patients.
