What You'll Do11
- 1Coordinate and track medical behavioral health telehealth appointments
- 2Assist members with scheduling and appointment reminders
- 3Escort members to onsite appointments
- 4Provide recovery-oriented peer support and encouragement
- 5Support engagement in treatment and recovery programming
- 6Communicate member needs to clinical and operational teams
- 7Maintain accurate documentation and ensure compliance
- 8Identify and address barriers to care
- 9Drive continuous improvement in care coordination processes
- 10Collaborate with multidisciplinary teams
- 11Promote wellness and independence among members
Requirements11
- 1Must be 21 years of age or older
- 2High school diploma or GED required
- 3Behavioral Health Technician designation required
- 4Peer and Recovery Support Specialist certification required
- 5Strong communication and organizational skills
- 6Proficiency with Microsoft Office and EHR systems
- 7Ability to work independently and collaboratively
- 8Experience in substance use treatment or integrated healthcare settings
- 9Familiarity with Kipu electronic health records preferred
- 10Valid driver's license and MVR clearance
- 11Background check and safety certifications required
