Overview

Certified Peer Specialist leading supportive counseling and time-limited intensive care coordination for high-risk clients. This is a full-time position in Manhattan where you will develop client skills, link resources, and provide advocacy. We seek a compassionate professional dedicated to recovery and wellness.

What You'll Do11

  • 1Engage clients at hospital bedside to deliver program overview and conduct intake assessments
  • 2Develop rapport with clients to improve health and wellness outcomes
  • 3Participate in weekly team meetings to discuss referrals and client updates
  • 4Create WRAP and Relapse Prevention Plans to address triggers and cravings
  • 5Act as role model educating clients on self-help techniques and coping strategies
  • 6Provide transportation training and escort clients to appointments
  • 7Assist clients in building community support systems and networks
  • 8Offer hands-on assistance with activities of daily living
  • 9Deliver training in self-advocacy and teach symptom management skills
  • 10Secure access to healthcare social services and community resources
  • 11Collaborate with multidisciplinary team members to achieve optimal health outcomes

Requirements6

  • 1Lived experience receiving mental health services
  • 2Must hold Certified Peer Specialist certification in New York State
  • 3High School diploma or GED
  • 4Excellent written and verbal communication skills in English
  • 5Basic computer proficiency and data entry abilities
  • 6Valid New York State peer specialist license
