Overview

Pharmacist role owning oncology patient medication management at a nationally recognized medical center. You will work remotely with care teams under a CPA to assess patients, order and monitor treatment plans, and resolve medication issues. This hybrid, California-based position blends clinical practice with collaborative care and interdisciplinary teamwork, delivering high‑quality, evidence‑based medication management. You’ll experience a patient‑centered environment and build relationships with patients and providers while contributing to quality improvement efforts.

What You'll Do5

  • 1Build comprehensive medication management plans and reconcile therapies to optimize outcomes
  • 2Partner with physicians and specialists to order individualized care plans under a CPA and ensure seamless medication coordination
  • 3Monitor therapy effectiveness and safety, identify opportunities for optimization, and intervene as needed
  • 4Support quality‑improvement and REMS initiatives within the oncology program
  • 5Assist operations with complex prior authorizations, workflow improvements, patient education, and ongoing clinical development

Requirements6

  • 1Hold a Doctor of Pharmacy (PharmD)
  • 2Must be licensed pharmacist in California
  • 3Completion of PGY2 Oncology Residency or PGY1 Ambulatory Care Residency with at least 3 years of clinical oncology experience
  • 4At least 3 years of oncology pharmacy experience, including 1 year under a Collaborative Practice Agreement (CPA)
  • 5Strong communication, organization, and technology skills
  • 6Empathy, integrity, and a team‑first mindset
