What You'll Do11

  • 1Uphold Kaiser Permanente policies and procedures Principles of Responsibilities state federal and local laws
  • 2Conduct patient medication history inquiries upon referral from physicians and other medical personnel
  • 3Synthesize treatment plans based on diagnoses incorporating drug therapy and lifestyle modifications
  • 4Identify resolve and prevent medication related problems such as adverse drug reactions drug interactions and inappropriate dosing
  • 5Initiate adjust monitor and recommend changes in drug therapy using patient interviews objective monitoring parameters and medical records
  • 6Coordinate laboratory monitoring of drug therapy including serum drug levels electrolytes and other approved tests
  • 7Provide patient education on disease states side effects compliance and special instructions
  • 8Assure ongoing performance improvement of pharmaceutical care services
  • 9Perform drug utilization reviews comprehensive medication reviews and medication reconciliations
  • 10Operate pharmacy computer systems and other equipment documenting information according to policies and regulatory requirements
  • 11Support collaborative labor management partnership environment performing other duties as required

Requirements11

  • 1Minimum 1200 hours experience in last five years working under collaborative drug therapy management protocol
  • 2Board Certified Oncology Pharmacist (BCOP) or completed PGY2 oncology practice residency
  • 3California Pharmacist License and National Provider Identifier required at hire
  • 4Basic Life Support certification within three months of hire
  • 5Pass standardized competency assessment
  • 6Experience performing aseptic techniques and preparing sterile IV products and hazardous agents
  • 7Education degree in pharmacy meeting licensing requirements
  • 8Doctorate of Pharmacy PharmD preferred
  • 9PGY-2 clinical pharmacy specialty residency or fellowship
  • 10Board certification in relevant practice area
  • 11Primary location California Woodland Hills Woodland Hills Medical Center
