What You'll Do11

  • 1Lead medication therapy management through active team collaboration
  • 2Select personalized drug therapies and implement patient-specific regimens
  • 3Conduct laboratory monitoring and provide comprehensive medication education
  • 4Utilize population health strategies to improve outcomes and reduce readmissions
  • 5Facilitate patient education on self-management and medication safety
  • 6Maintain protocol certification for diabetes anticoagulation and transition care
  • 7Assist pharmacy residents and students in clinical practice
  • 8Perform disease state management and adverse drug event prevention
  • 9Develop evidence-based guidelines and analyze medication-use data
  • 10Communicate clinical recommendations to providers and escalate complex cases
  • 11Adhere to Pharmacy and Therapeutics Committee protocols and standards

Requirements10

  • 15+ years building ambulatory care medication management systems
  • 2Spark and Airflow experience with ETL pipelines
  • 3AWS certified cloud infrastructure knowledge
  • 4BCCP board certification preferred
  • 5PhD from ACPE accredited pharmacy school
  • 6Advanced Cardiac Life Support certification required
  • 7Board Certified Ambulatory Care Pharmacist preferred
  • 8Certified Diabetes Care and Education Specialist preferred
  • 9Registered Pharmacist license in Arizona
  • 10Experience with BC-ADM and CDCES credentials valued
