What You'll Do8
- 1Provide comprehensive medication management including reconciliation dose optimization and adherence support
- 2Partner with physicians and specialists to create individualized care plans under a CPA
- 3Monitor therapy effectiveness and safety while identifying optimization opportunities
- 4Support quality improvement and REMS initiatives
- 5Offer patient education and maintain ongoing clinical development
- 6Assist with complex prior authorizations and workflow improvements
- 7Collaborate with interdisciplinary teams to educate patients and coordinate care
- 8Drive operational recommendations and improve prescribing processes
Requirements8
- 1Doctor of Pharmacy PharmD degree required
- 2Licensed pharmacist in Oregon
- 3Completion of a PGY2 Ambulatory Care Residency
- 4At least 1 year of ambulatory practice under a Collaborative Practice Agreement
- 5Minimum 3 years of ambulatory practice managing specific disease states
- 6Strong communication and technical skills
- 7Empathy integrity and team-first mindset
- 8California residency or willingness to relocate to Oregon
