Overview
The Mobile Crisis Team Clinician leads integrated crisis response for Catholic Charities Brooklyn and Queens. This full-time position operates 24/7 across Queens and Brooklyn delivering immediate mental health support and connecting clients to long-term care. You will develop crisis plans and coordinate services to prevent hospitalizations and decompensation.
What You'll Do8
- 1Provide immediate telephonic assessment to determine crisis level and safety risks
- 2Conduct face-to-face evaluations when required to stabilize acute situations
- 3Make referrals for emergency care or alternative treatment options
- 4Create or update personalized crisis plans within Person Centered Frameworks
- 5Coordinate discharge planning and community re-entry for individuals transitioning from hospitalization
- 6Monitor progress across multiple care levels and maintain family communication
- 7Implement TA Inc protocols for emergency services and medication administration
- 8Collaborate with clinical teams to deliver integrated behavioral health services
Requirements6
- 1Master’s degree in Social Work or related field with NYS LMSW or LHMC preferred
- 2Minimum one year crisis management experience in assertive outreach or emergency settings
- 3Bilingual Spanish English or other high-demand language proficiency
- 4Ability to travel throughout all five boroughs daily
- 5Flexible availability including weekends and evenings
- 6Strong interpersonal skills for client engagement and family collaboration
