What You'll Do9
- 1Build daily operations of the social services team, including policies, procedures, and care coordination with other clinical providers.
- 2Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting professional growth.
- 3Oversee assessment, planning, and evaluation of social services care, ensuring clinical documentation meets state, federal, and ACHC standards.
- 4Partner with the Executive Director on annual department budget and resource planning.
- 5Build strong knowledge of community resources and connect patients and referral sources to the right support.
- 6Plan in-service training and continuing education to keep the team’s skills sharp.
- 7Contribute to quality improvement initiatives and identify new ways to better serve patients.
- 8Perform other duties as required for patient services.
- 9Flexible Schedule and potential for Full-Time conversion based on performance.
Requirements8
- 1An MSW from an accredited program.
- 2Active California license as an LCSW.
- 3At least 3 years of management experience in home care.
- 4Strong knowledge of healthcare regulations, ACHC standards, and business operations.
- 5Excellent communication skills and ability to build trust with patients, families, staff, and the community.
- 6Self-directed, organized, and flexible approach with initiative and collaboration.
- 7Ability to Convert to Full-Time Based On Performance
- 8Per diem availability with a rate of $100 - $120 per visit
