Medical Social Worker, Home Health & Community-Based Care
Overview
Master of Social Work trained professionals join a multi disciplinary team delivering home health visits at scale. You will partner with clinical staff to address social and emotional factors impacting health outcomes. Work across designated territories, advocate for clients, coordinate community resources, and document care using EMR on a mobile device. This role emphasizes independent time management within a values driven, compassionate Bayada environment.
What You'll Do9
- 1Build and conduct home visits across designated territories to assess social needs and plan interventions
- 2Identify community resources and develop referrals to support patient safety, independence, and well being
- 3Advocate for clients and families to navigate community systems and care options
- 4Partner in developing the total plan of care with BAYADA clinicians and multidisciplinary team
- 5Educate patients and families about resources to promote health and independent living
- 6Document observations and interventions accurately in the EMR on a touch pad tablet
- 7Maintain contracts with public and private agencies to expand patient resources
- 8Collaborate with team to ensure timely and patient centered care
- 9Support care planning and coordination to align with client goals
Requirements9
- 1Must be licensed by the NJ Board of Social Work Examiners at the LMSW or LISW level
- 2Masters in Social Work with at least 2 years of community based experience
- 3Accreditation by the Council on Social Work Education
- 4Home care experience a plus
- 5Ability to read write and communicate effectively in English
- 6Ability to work independently and manage time effectively
- 7Strong interpersonal skills
- 8Solid computer skills with prior EMR experience preferred
- 9Ability to travel to cases as assigned
