Overview
BAYADA seeks an energetic Licensed Medical Social Worker MSW to provide home hospice care in Philadelphia. The role involves visiting clients across Camden and Burlington counties delivering skilled nursing and palliative care. This position offers a chance to support families while remaining at home.
What You'll Do11
- 1Conduct home visits within designated geographic territories
- 2Assess social and emotional factors influencing client health outcomes
- 3Perform clinical social evaluations and design targeted interventions
- 4Refer community resources to meet practical and environmental needs
- 5Advocate for clients and families navigating complex systems
- 6Collaborate with multidisciplinary teams to develop comprehensive care plans
- 7Maintain contracts with public and private agencies as patient resources
- 8Educate patients and families about community resources for safety and independence
- 9Document patient care using state-of-the-art digital tablets
- 10Lead initiatives to improve care coordination and outcomes
- 11Travel to assigned case locations as needed
Requirements10
- 1Licensed by NJ Board of Social Work Examiners at Master level
- 2Master of Social Work with minimum two years community-based experience
- 3Accreditation by CSWE confirmed via transcript or degree
- 4Proficiency in reading writing communicating effectively in English
- 5Ability to work independently managing time effectively
- 6Strong interpersonal communication and interpersonal skills
- 7Solid computer proficiency including EMR systems preferred
- 8Capacity to travel to client locations
