What You'll Do12

  • 1Build and oversee safe, high-quality home health services for a large patient population
  • 2Coordinate and mentor field clinicians and resolve clinical concerns with timely guidance
  • 3Review and update individualized care plans ensuring accuracy and current plans of care
  • 4Triage calls from patients and caregivers, prioritize clinical issues, and escalate as needed
  • 5Enter physician orders into EMR accurately and promptly
  • 6Communicate with physicians, providers, clinicians, patients, and caregivers to ensure continuity of care
  • 7Monitor clinical documentation for accuracy and timeliness, conduct chart audits, and address deficiencies
  • 8Ensure compliance with home health regulations, standards, and company policies
  • 9Assist with onboarding and training of new clinical staff
  • 10Coordinate medical supplies and equipment, maintaining OSHA and safety standards
  • 11Support daily operations between office and field staff and identify improvements
  • 12Highlight operational or clinical issues to leadership for timely resolution

Requirements8

  • 1Active California RN License
  • 2Home Health experience
  • 32+ years of supervisory experience
  • 4BLS Certification
  • 5Excellent organizational and communication skills
  • 6Strong computer proficiency (Apple products & EMR systems)
  • 7Ability to multitask and manage high call volumes
  • 8Associate Degree in Nursing (Required; BSN Preferred)
