What You'll Do5
- 1Lead patient care coordination across home health to achieve outcomes within 90 days
- 2Design and implement nursing workflows that ensure regulatory compliance and high-quality care RN services
- 3Partner with clinicians to develop and monitor care plans that improve patient satisfaction and reduce hospital readmissions
- 4Drive the recruitment, onboarding, and development of nursing staff to maintain high performance
- 5Collaborate with interdisciplinary teams to align on patient care goals and operational efficiency
Requirements5
- 1Graduate from an approved school of professional nursing and currently licensed as a registered nurse in the state of operation
- 23-5 years experience as a RN
- 3Experience managing RN services in the home, community, or clinic (preferred)
- 4Licensed to practice as a registered nurse in the state of operation and valid CPR certification
- 5Licensed driver with a vehicle insured in accordance with state requirements and able to travel to all business locations
Salary Insight
$110 - $135k per year
