Overview

You own the full lifecycle of medical records for a busy Houston clinic, from intake to archival. You track both physical files and electronic health records in EPIC, ensuring accuracy and compliance with HIPAA. You work alongside a team of 5 clerks and report directly to the Health Information Manager. Your primary tool is MS Excel for audits and tracking, but you also train new staff on record systems. This role offers a clear path to RHIT certification sponsorship.

What You'll Do7

  • 1Organize physical and electronic patient files, using EPIC to map records to correct folders and storage.
  • 2Enter patient demographics, diagnoses, and treatment codes into the EMR system within 24 hours of visit.
  • 3Retrieve and deliver requested charts to physicians, nurses, and external auditors, logging each request in MS Excel.
  • 4Archive records older than 5 years, labeling boxes with HIPAA-compliant retention dates and storing them in the offsite warehouse.
  • 5Run monthly audits on 200+ records to find missing signatures or misfiled entries, correcting errors before external reviews.
  • 6Train 2 new clerks per quarter on filing procedures and EPIC navigation, using a checklist I maintain.
  • 7Purge expired records per state law, shredding documents after verifying they are no longer needed for active litigation.

Requirements7

  • 11+ years in a medical records or health information management role, with hands-on EPIC experience.
  • 2High school diploma or equivalent, plus a certificate in Medical Coding or Health Information.
  • 3Working knowledge of HIPAA privacy rules and record retention schedules.
  • 4Proficiency in MS Excel for tracking audits and log reports.
  • 5Ability to lift up to 30 lbs for moving archive boxes.
  • 6Detail-oriented with a high accuracy rate in data entry, meeting a 98% accuracy benchmark on audits.
  • 7Strong communication skills for training staff and assisting external auditors.
