Requirements5
- 1Bachelor’s degree in Social Work Human Services Public Health Urban Planning or related field
- 2Minimum 2 years case management housing coordination experience
- 3Knowledge of Medicaid regulations eligibility requirements
- 4Experience with diverse populations including individuals with disabilities and low-income families
- 5Valid driver’s license and reliable transportation for client meetings
