Overview

Manage the RAI Process from resident admission to discharge while maintaining clinical compliance and securing funding from Medicare Medicaid and Managed Care pay sources. Oversee MDS documentation and case mix optimization to ensure proper reimbursement.

What You'll Do11

  • 1Build and lead the RAI process ensuring accurate documentation and timely completion
  • 2Develop individualized care plans reflecting resident status
  • 3Oversee resident care data collection using specialized tools
  • 4Coordinate assessments and services across clinical departments
  • 5Act as liaison to rehabilitation teams
  • 6Prepare for audits and surveys implementing corrective actions
  • 7Manage managed care clinical authorizations and justifications
  • 8Complete state-required forms accurately
  • 9Provide ADL training and support
  • 10Manage restorative nursing programs and case mix adjustments
  • 11Attend daily PPS/OBRA meetings and weekly standards of care meetings

Requirements11

  • 15+ years building ETL pipelines with Spark and Airflow
  • 23-5 seasons of experience in long term care
  • 35+ years completing MDS documentation in a timely accurate manner
  • 42 years experience with Medicare Case Management and Medicaid reimbursement forms
  • 5Category 1 risk of infectious disease exposure with proper training
  • 6Excellent communication and teaching skills for residents and staff
  • 7Proficiency in computer systems and documentation software
  • 8Ability to follow oral and written instructions independently
  • 9Strong teamwork skills for cooperative work environment
  • 10Good working knowledge of Texas Case Mix and reimbursement processes
  • 11Respectful interaction with residents families and colleagues
