Overview

You will own the entire resident assessment process for a skilled nursing facility in Tampa, FL. You will lead MDS completion, RAI coordination, and reimbursement accuracy across Medicare, Medicaid, and OBRA programs. You will work with the interdisciplinary care team, including rehab, nursing, and administration, to ensure timely and compliant assessments. This role offers the chance to shape QA processes and survey readiness from day one.

What You'll Do9

  • 1Direct the RAI process from admission to discharge, ensuring all MDS assessments meet CMS and state guidelines.
  • 2Chair daily case management, weekly UR, and monthly triple check meetings to keep assessments on track.
  • 3Monitor QI/QM reports and launch corrective actions to close gaps in resident outcomes.
  • 4Build and update policies that govern the MDS process, including the RAI Manual.
  • 5Partner with the Director of Rehab to set accurate PPS pathways and ARDs for Medicare Part A residents.
  • 6Complete entry trackers, discharge assessments, and significant change assessments within 14-day windows.
  • 7Validate MDS transmission reports, resolve fatal errors, and process modifications per CMS policy.
  • 8Provide PPS pathways to billers and case managers to support accurate reimbursement and managed care updates.
  • 9Prepare for state and federal audits, including documentation for Medicare certifications.

Requirements8

  • 12+ years as an MDS Coordinator in a skilled nursing facility.
  • 2Active, unencumbered RN license in Florida or compact state.
  • 3Knowledge of RAI process, MDS 3.0, and OBRA regulations.
  • 4Proven ability to lead interdisciplinary teams and drive compliance.
  • 5Experience with CMS correction policies and PPS pathways.
  • 6Bachelor's degree in nursing from an accredited program.
  • 7Strong organizational skills to manage multiple deadlines.
  • 8Physical ability to lift up to 45 pounds as needed.
