MDS Coordinator (LPN/RN) Skilled Nursing
Overview
You will own the full MDS assessment lifecycle at a skilled nursing and rehabilitation center, coordinating resident evaluations for 100+ beds and driving Five-Star quality ratings. You will work with the interdisciplinary care team, including RNs, LPNs, and therapists, to ensure regulatory compliance. What You'll Do8
- 1Conduct MDS 3.0 assessments and coordinate care planning within regulatory timeframes.
- 2Submit accurate MDS records to the State Repository per RAI Manual guidelines.
- 3Drive the interdisciplinary team to set assessment reference dates and review schedules.
- 4Update resident care plans after quarterly reviews or status changes using PointClickCare.
- 5Analyze Quality Measures reports to maintain Five-Star ratings.
- 6Educate nursing staff on MDS documentation and Care Area Assessments.
- 7Participate in state surveys and AHCA inspections, ensuring compliance.
- 8Safeguard resident privacy with HIPAA-compliant data handling.
Requirements6
- 1Active LPN or RN license in Florida.
- 2Nursing degree from an accredited program.
- 32+ years in skilled nursing, long-term care, or hospital settings.
- 4Working knowledge of MDS 3.0, RAI, and CMS regulations.
- 5Leadership skills to coordinate interdisciplinary teams.
