LPN Case Manager
Overview
Lead patient case coordination overseeing care plans and scheduling for 4,300 caregivers in NYC and Long Island. This role sets nurse assignments determines service hour reviews and ensures coverage integrity. You will resolve missed visits and escalate issues to clinical leadership.
What You'll Do11
- 1Build care coordination processes aligned with agency standards
- 2Design nurse schedules based on patient needs and authorizations
- 3Review clinical documentation to support authorized services
- 4Identify coverage gaps and implement solutions before they impact care
- 5Escalate patient condition changes to clinical leadership promptly
- 6Maintain accurate EMR and agency system records meeting LHCS standards
- 7Provide direct patient and caregiver support resolving concerns immediately
- 8Coordinate with clinical and intake teams rather than working around them
- 9Manage multiple priorities in a fast-paced environment
- 10Demonstrate strong communication skills across diverse stakeholders
- 11Utilize EMR and scheduling platforms efficiently
Requirements7
- 1Current NYS LPN license in good standing
- 2Experience in home care case management or scheduling within LHCSA settings
- 3Familiarity with HHA Exchange or similar scheduling platforms
- 4Knowledge of Medicaid and home care documentation standards
- 5Bilingual English Spanish proficiency
- 6Ability to operate under pressure with a mobile phone
- 7Strong organizational skills and attention to detail
