Requirements7

  • 1Current NYS LPN license in good standing
  • 2Experience in home care case management or scheduling within LHCSA settings
  • 3Familiarity with HHA Exchange or similar scheduling platforms
  • 4Knowledge of Medicaid and home care documentation standards
  • 5Bilingual English Spanish proficiency
  • 6Ability to operate under pressure with a mobile phone
  • 7Strong organizational skills and attention to detail
