What You'll Do9
- 1Coordinate and oversee participant care plans using holistic interdisciplinary approach
- 2Review clinical assessments completed by field staff to ensure quality and accuracy
- 3Develop, implement, and monitor individualized care plans
- 4Collaborate with healthcare providers, caregivers, and community resources to coordinate services
- 5Monitor participant progress and update care plans as needed
- 6Ensure compliance with organizational policies and regulatory standards
- 7Provide exceptional customer service to participants families and healthcare partners
- 8Maintain accurate and timely documentation
- 9Participate in interdisciplinary meetings and case conferences to support quality care outcomes
Requirements8
- 1Active New York State Licensed Master Social Worker license required
- 2Master's Degree in Social Work from NYSED approved program
- 3Minimum of 2 years administrative experience in CHHA managed care home health clinic settings
- 4Minimum of 1 year experience working with frail elderly population
- 5Strong customer service experience
- 6Excellent communication organizational and care coordination skills
- 7Preferred Managed Long-Term Care insurance experience
- 8Supervisory experience
