LPN Nurse Care Manager, Special Medical Preventive Services
Overview
You will own health coordination for families with complex medical needs in the Special Medical Preventive Services program at Cardinal McCloskey Community Services (CMCS). You will work onsite in New York, NY for 21 hours per week, conducting home visits and serving as a medical liaison. What You'll Do10
- 1Assess family health concerns during home visits and provide education and support.
- 2Deliver services per ACS, OCFS, and state mandates.
- 3Lead group health education activities to empower families.
- 4Serve as the medical liaison between families, hospitals, clinics, and community providers.
- 5Coordinate all family health services for seamless communication.
- 6Guide families on preventive, curative, and rehabilitative health matters.
- 7Explain medical recommendations and follow-up care for chronic or communicable conditions.
- 8Develop community health nursing plans addressing maternal and child health, chronic illness, and disease prevention.
- 9Participate in referral and follow-up processes for infants, children, adolescents, and adults.
- 10Collaborate with social workers and providers to ensure comprehensive care.
Requirements4
- 1Current New York State LPN Licensure.
- 21+ years providing healthcare management or community-based nursing.
- 3Bilingual preferred.
- 4Ability to work 21 hours per week in a part-time onsite role.
