What You'll Do8
- 1Conduct initial patient assessments and document findings in EPIC.
- 2Administer medications and treatments per physician orders, using IV pumps and other equipment.
- 3Monitor vital signs with CPR and AED readiness, reporting changes to the care team.
- 4Assist with daily living activities, including bathing and mobility using Hoyer lifts.
- 5Deliver wound care and injections following clinical protocols.
- 6Educate families on disease management and medication safety.
- 7Maintain accurate patient records in Kinnser or HomeCare HomeBase.
- 8Collaborate with RNs and therapists to adjust care plans.
Requirements9
- 11+ years of LPN experience in home health, long-term care, or hospital setting.
- 2Active New York LPN license in good standing.
- 3Current CPR certification (BLS for Healthcare Providers).
- 4Valid driver's license and reliable transportation to travel to patient homes.
- 5Experience with EHR systems such as Epic or HomeCare HomeBase.
- 6Ability to lift and transfer patients up to 50 pounds.
- 7Strong clinical assessment and critical thinking skills.
- 8Comfortable with home visits in varied environments across NYC boroughs.
- 9Flexibility to work rotating weekends and holidays.
