LMSW Client Care Coordinator Supportive Housing
Overview
The role involves leading trauma-informed counseling and support services for families in supportive housing. Candidates will conduct assessments, provide counseling, connect families to resources, and collaborate with teams to ensure stability and well-being.
What You'll Do9
- 1Conduct psychosocial assessments to identify clinical emotional and service needs of individuals and families
- 2Provide short-term counseling crisis intervention support groups and workshops for residents
- 3Connect clients to community-based mental health substance use and domestic violence services
- 4Collaborate with internal teams and external providers to coordinate care and ensure continuity
- 5Identify and support high-risk clients including survivors of domestic violence mental health challenges and families involved with ACS
- 6Partner with parents schools healthcare providers early intervention programs and community agencies to address childrens needs
- 7Facilitate therapeutic play groups workshops and trauma-informed programming for adults and children
- 8Participate in case conferences team meetings on-call rotations trainings and other activities supporting client success
- 9Lead efforts to improve family healing growth and thriving within supportive housing program
Requirements10
- 15+ years building ETL pipelines with Spark and Airflow
- 2CPC Card certification
- 33-5 seasons of experience
- 4AWS Certified
- 55+ years building ETL pipelines with Spark and Airflow
- 6BPMN modeling proficiency
- 7Lean Six Sigma green belt
- 8Project Management certification
- 9Advanced Excel skills
- 10Case management software expertise
