What You'll Do8

  • 1Deliver therapy and behavioral health services to adult and senior patients in skilled nursing and rehabilitation settings within your first 90 days.
  • 2Customize treatment plans with an integrated care team using EMR for efficient documentation.
  • 3Manage your caseload independently while prioritizing patient outcomes and continuity of care.
  • 4Collaborate with facility staff and physicians to coordinate holistic care for each patient.
  • 5Document patient progress, interventions, and outcomes accurately in the EMR system.
  • 6Participate in clinical supervision and training in geropsychology to enhance specialty skills.
  • 7Adapt evidence-based therapeutic approaches to meet the needs of diverse geriatric populations.
  • 8Build rapport with patients, families, and facility teams to foster trust and improve engagement.

Requirements7

  • 1Master's degree in Counseling from an accredited program.
  • 2Active LPC license in the state of practice (Georgia preferred).
  • 3Experience with adult or geriatric populations, ideally in long-term care or rehab settings (new graduates encouraged).
  • 4Strong clinical judgment, empathy, and verbal/written communication skills.
  • 5Ability to work independently and manage documentation within EMR systems.
  • 6Commitment to professional development and ongoing CEU training.
  • 7Availability for weekday hours between 7 AM and 7 PM, with flexible scheduling.
