Overview
Delta-T Group seeks a Housing Case Manager for immediate start in Boston Massachusetts. This temporary position involves interviewing clients developing case plans monitoring progress and securing housing. The role requires strong communication skills and ability to work independently.
What You'll Do7
- 1Interview and assess all shelter residents for five days
- 2Conduct intake assessments and identify resource needs
- 3Create individualized case plans with weekly progress monitoring
- 4Manage electronic client records in HMIS and complete documentation
- 5Provide referrals and assist with applications for housing and benefits
- 6Build relationships with community partners and housing agencies
- 7Maintain professional relationships with landlords and property managers
Requirements5
- 1College degree or high school diploma/GED with one to two years experience in health services with homeless populations
- 2Spanish language skills preferred
- 3Basic proficiency in Windows Word and Google Email
- 4Ability to obtain and complete crisis intervention training
- 5Minimum one year experience in behavioral health case management
