Overview
The Hospice Registered Nurse Case Manager leads skilled nursing care delivery across Robertson Cheatham Davidson Montgomery counties. This full time position requires on call rotations Monday Friday. You will develop care plans coordinate patient care ensure documentation accuracy and educate families about disease processes. Outcomes are measured within the first ninety days. This role differs by focusing on comprehensive case management within a hospice setting.
What You'll Do11
- 1Deliver and document skilled nursing care following physician orders
- 2Continuously assess patient conditions and report changes promptly
- 3Maintain accurate records of all nursing activities and interventions
- 4Create and update nursing care plans based on patient needs
- 5Implement appropriate nursing actions to prevent risks
- 6Follow infection control protocols aligned with OSHA standards
- 7Educate patients and families on disease management and self care
- 8Participate in ongoing education to maintain clinical competencies
- 9Ensure compliance with state licensing board regulations and accreditation requirements
- 10Coordinate care with interdisciplinary teams
- 11Manage patient flow and schedule within assigned territory
Requirements10
- 1Graduate of an accredited diploma associate or baccalaureate school of nursing
- 2Current unrestricted RN license in Tennessee
- 3Active CPR certification
- 4Valid driver’s license with proof of insurance
