Overview
The Registered Nurse RN Care Manager leads patient care across multiple locations supporting hospice services. You will own the Plan of Care for a caseload while collaborating with interdisciplinary teams. This role offers a unique opportunity to make a meaningful impact in a faith-driven community setting.
What You'll Do11
- 1Build and maintain relationships with patients families and care providers
- 2Design and implement individualized care plans aligned with patient goals
- 3Coordinate on-call rotations for emergency clinical needs
- 4Administer medications and therapies according to prescribed protocols
- 5Educate patients and caregivers on disease progression and symptom management
- 6Analyze outcome data to improve care delivery
- 7Process resource orders and manage supply inventory
- 8Maintain accurate documentation in electronic health records
- 9Collaborate with physicians to adjust treatment plans
- 10Lead initiatives to enhance patient satisfaction and outcomes
- 11Support staff through training and professional development
Requirements11
- 15+ years experience as a Registered Nurse in hospice or palliative care settings
- 2Bachelor's degree in Nursing preferred
- 3Current RN license and BLS CPR certification required
- 4Valid driver's license and auto insurance
- 5Certified Hospice and Palliative Nurse credential preferred
- 6Proven ability to work independently and lead projects
- 7Strong analytical skills with data interpretation experience
- 8Excellent communication abilities in English
- 9Physical stamina to perform manual tasks and lift light loads
- 10Commitment to holistic patient care philosophy
- 11Experience with Microsoft Office Suite and EMR systems
